There is one at Frankfurt Hahn airport. I didn't inspect it to see what image it creates, but there was a girl asking her father for €1.05

No, I didn't test the machine, but if I remember correctly it makes four different coins. I was there for some days, and when I found the machine I remembered once I saw this thread.

I found one today, at the Alcobaça Monastery. It can be found at the entrance to the sacristy, seen here. It asks for a 5 cent to be smashed, and 1€ for Our Lord Jesus Christ.
At another altogether different sort of "temple" - the Sport Lisboa e Benfica Stadium in Lisbon - a coin smasher can be found at the stadium's commercial gallery, close to the Adidas shop and the club's shop. I can't believe I never thought of taking a picture of it, or just mentioning it here.
