I'll make two categories

1. Before Euro

Sweden
Denmark
The Netherlands
Belgium
Germany
Luxemburg
Austria
Switzerland
Italy
France
Spain (Canary Islands)
Greece
Estonia

2. Since January 1st, 2002

Sweden
Denmark (Only Copenhagen Airport )
United Kingdom
Germany ( Lübeck, Hamburg, Cologne, Munich, Nuremburg, Berlin ) as you can see from here

Dot from Berlin is missing because I lost the paper where I had written serialnumbers of about 10 notes. I was on the very bad mood when I noticed that back in Finland.

Next summer I've planned to travel around Europe my whole summer holiday--> 1 month. I hope that my map is going to look very much more interesting after that.

Never been outside Europe

Spain Image (several times) - Madrid, Sevilla, Ayamonte, Barcelona, Bilbao, Benidorm...
France (6x) Image - Paris, Bordeaux, Lyon, Lourdes, Marseille, Taizé and some others I can't remember right now...
Monaco (2x) Image - Monte Carlo
Italy (2x) Image - Milano (both of the times - and I still think it's an awful city!), Padua and Venezia (had dinner there at a Pizzaria the evening of January 1st 1999 - it was wonderful).
Switzerland (2x) Image - Genéve (both times)

Portugal Image - I live here, but it's surprising how little I know of this country. I've been north of Coimbra about three times in my life (and I've never been to Coimbra). Neither have I ever visited Madeira or Açores. I've been to Porto once. Shame on me, I know. Well, that's life, Jim.
I also need to add ImageSweden to my list (Göteborg). And ten hours in France ;) (CDG airport :P)

Except for Albania, Cyprus, Ireland, Malta, Moldova and Ukraine I visited every European country. Best of them all I found Iceland but I have a weak spot for the Baltic region as well.

Outside Europe I've only been once. The train took me from Moscow through Mongolia to China and the plain from China to Thailand and Australia.

Well.. I haven't seen my home country practically at all, either.
I've once been as north as Lieksa. Was there for one day. Even that's far less than halfway the country :D
Most of my life I've been sitting in the capital city area and looked at trams and metros rolling around :))
I kind of hate this country. Too cold for me. And I hate snow.
And my GF said she's gonna make me move to Lapland some day and live there the rest of my life :D

Portugal (I live here, and I've traveled thoroughly, apart from having worked as a trekking guide; still failing to visit 5 of the 9 Azores islands, Madeira, and the northernmost part of Trás-os-Montes)
Spain (Barcelona, Sevilla, Vigo, Cáceres, Mérida, Salamanca, Picos de Europa, some towns in Galicia, Catalunya and Andalucía; plus Madrid's train station)
France (Paris, Compiègne, some towns in Languedoc-Roussillon, including the wonderful Castelnou, some towns by train only)
Italy (Roma, Venezia, Bologna, Padova, Napoli, Assisi, Siena, Milano, the whole Dolomites and a bunch of other small towns)
United Kingdom (London and Reading several times, Edinburgh, a lot of towns from Cornwall to Wales, including Llanfairpwllgwyngyllgogerychwyrndrobwllllantysiliogogogoch, to Scotland)
Germany (Köln, Freiburg, some other towns and places)
Czech Republic (Praha, some towns all around Bohemia, including the amazing České Budějovice, some towns in Moravia)
Slovakia (the beautiful High Tatras and some towns elsewhere)
Belgium (only Bruxelles/Brussel)
Hungary (Budapest only)
Austria (Wien, Melk, Linz, Innsbruck)
Switzerland (Interlaken only)
Luxemburg (Luxembourg, of course)
